A great opportunity exists for Aboriginal and/or Torres Strait Islander peoples from around Australia to participate in a 6 week program in Sydney NSW to assist you in preparing to join either the Navy, Army or Air Force.

We are looking for participants who are at least 17 years old and have completed or are willing to complete the Australian Defence Force ‘Your Opportunities Unlimited (YOU) Session”.

If you are eligible for any Centrelink or Abstudy benefits and wanting to join the program your return flights to Sydney, accommodation and meals during the program will be FREE.

This full time program will start on 28th October 2011 in Sydney for 6 weeks with additional learning from home in 2012.

ACCOUNTABILITY

Responsible to the WAMS Board of Directors via the Chief Executive Officer

JOB SUMMARY

• Work with FCPHC Aboriginal Health Workers to provide an outreach Family Centred Primary Health Care to Aboriginal people in Goodooga, Pilliga, Gingie and Namoi.
• Co-ordinate the work the of the Medical and Allied Health Care Teams to assist in the provision of  the Family Centred Primary Health Care to those outlying areas  covered by this program.
• Lead a multi-disciplinary team in the provision of high quality Family Centred Primary Health Care to outlying communities.

QUALIFICATIONS AND EXPERIENCE

ESSENTIAL:

• Registered Nurse List A
• Current NSW Driver’s Licence
• Experience providing Primary Health Care across a range of settings
• Previous experience in;

- working with Aboriginal communities

- program planning

- good written and oral communication skills

• Demonstrated ability to lead a Multi-disciplinary Primary Health Care Outreach Team

DESIRABLE:

• Community health/education/mental health qualifications
• Report and submission writing skills

·      Computer literacy with experience in Works for Windows

• Midwifery and/or Early Childhood Qualification

DUTIES:

• Maintain confidentiality and privacy in all matters relating to WAMS at all times
• Work with members of the Family Centred Primary Health Care Team to

- assess community needs, resources available and set priorities

- plan, implement and evaluate programs which are designed to meet the     needs of the community

• Contribute to the WAMS continuous quality improvement process and ensure that the Family Centred Primary Health Care service works within this framework at all times
• Maintain statistics in accordance with WAMS performance indicators
• Represent WAMS and the local community at the national, state regional and local level
• Liaise with the other staff of WAMS to ensure that;

- the safety of clients, staff and other users of WAMS is maintained

- resources are used effectively and efficiently

- adequate supplies are maintained

• Assist the Family Centred Primary Health Care General Practitioner and Allied Health Care staff to ensure a high quality of client care is provided to the communities serviced by this team.
• Work with Medical and Allied Health staff to ensure all Medicare entitlements are claimed by WAMS.
• Ensure that professional and personal development occurs whilst working with WAMS
• Liaise with all members of the community in the towns serviced by WAMS Family Centred Primary Health Care Team to ensure that each community receives a high quality of care

• Maintain accurate and current documentation of matters concerning clients and WAMS
• Work with the Family Centred Primary Health Care staff WAMS to ensure that;

- the safety of clients, staff and other users of WAMS

is maintained

- adequate supplies are maintained

JOB SUMMARY

· To establish and manage the Staying Home Leaving Violence Program within Walgett and surrounding areas.

· To work across a range of agencies and social housing providers to identify the housing and support needs of women and their children experiencing domestic/family violence and enhance their capacity to remain safe and secure in their homes

ESSENTIAL:

• 3 years experience working in a direct client contact role in health or human services
• Demonstrated experience in working with women and children affected by domestic/family violence in rural/remote communities
• Sound understanding of the function of government and non-government agencies that provide support or assistance to victims of domestic/family violence ( Police, Local Courts, Human Services-Community Services, Housing, local SAAP services)
• Demonstrated ability to liaise with service providers at a senior level
• Demonstrated experience in case management of people with high support needs
• Proven ability to work in an autonomous and accountable manner
• Excellent written, communication and computer skills
• Demonstrated understanding of the needs of and commitment to assisting Aboriginal communities
• Current Driver’s Licence

DESIRABLE:

· Tertiary qualifications in Social Work, Psychology, Behavioural Sciences or related areas

· To be of Aboriginal descent

· Knowledge of communities served by WAMS

DUTIES:

· Support women and children experiencing domestic/family violence to stay in their home

· Identify systemic problems and develop an appropriate service model to prevent homelessness when women and their children leave a domestic/family violence relationship

· Assess each client’s accommodation and /or support needs in a manner that enables the brokering of sustainable solutions to issues affecting women and their children experiencing domestic/family violence

· Identify key clients of women’s services/agencies and negotiate accommodation and support packages that match these client needs with available resources/services and work with them to develop innovative solutions to their issues

· Analyse case and client information trends and demographics to identify systemic blocks to client services and report these as required

· Develop strong lines of communication and networks to support women and their children experiencing domestic/family violence

· Work as a member of a multidisciplinary team
